Why are blank fields appearing in a per-user quarantine inbox on my Email Security Gateway?

All Email Security Gateway model 300s and above, all firmware versions.

An email's 'from' address can appear as blank in a user's quarantine inbox when that email does not contain a 'from' address in its headers. Also, since individual user whitelists and blacklists are built from the 'from' address in the headers, attempting to whitelist or blacklist a message with no 'from' address shown from a user's quarantine inbox will not add any new email address to that list.
